A reminder about filling some prescriptions
If you’re in the Premier PPO, Contribution HRA, Saver HSA, or Personalized Wellbeing Copay plan, you must use a Walmart, Walmart Home Delivery, Sam's Club or Walmart Specialty Pharmacy to get your pharmacy benefit. All other pharmacies are considered out-of-network, and in most cases, you'll pay the full retail price.
To be eligible, drugs must be listed on one of Walmart's formularies. These lists are maintained by OptumRx. They’re reviewed quarterly and often change. You can call OptumRx at 1-844-705-7493 to ask about a particular drug.
Some brand-name and other prescription drugs aren’t covered because generic or over-the-counter alternatives have proven to be just as effective. For more information about what is and is not covered, see the “Pharmacy Benefit” section of the Associate Benefits Book.
Some prescription drugs require prior authorization. Call OptumRx at 1-844-705-7493 to check which drugs are affected.
You can get a 30, 60, or 90-day supply of a maintenance medication when you use Walmart Home Delivery. You’ll pay three times the cost of a 30-day supply when you order a 90-day supply.
All specialty drugs under the Premier PPO, Contribution HRA, Saver HSA, and Personalized Wellbeing Copay plans must be purchased through Walmart Specialty Pharmacy. If a specialty drug is purchased from another network pharmacy, it will not be covered under the plan. You should call Walmart Specialty Pharmacy to ask about the availability of your prescription drugs under the plan.
If you take medications for certain inflammatory conditions, your plan covers these drugs differently. You can expect to pay as little as a $0 copay but you may have to switch to different, equally effective drugs that are on the preferred formulary. For non-preferred drugs you’ll pay a $25 copay for generic drugs, $100 copay for brand, and $250 for specialty. You must also use Walmart Specialty Pharmacy. If you later decide to join a Medicare drug plan, having “creditable coverage” under the Walmart plan will keep you from paying a higher premium under Medicare. For more information, see the Medicare Part D notice (PDF).
